Licensed Home Care Services Agency Programs
Maternal and Child Home Visiting Program
Public Health Nurses are available to provide comprehensive home based services, to families, that start during pregnancy or after a child is born. Home visits provide education, support and case management services and are focused on assisting parents to meet the challenges of parenting and to assure the healthy development of children.C
Childhood Lead Poisoning Prevention Program
Public Health Nurses provide individual case management and follow-up to children with elevated blood lead levels. Community education and outreach Environmental investigations are also available.
CHILDREN WITH SPECIAL HEALTH CARE NEEDS PROGRAM –
The Children with Special Health Care Needs Program (CSHCN) is a statewide public health program that provides information and referral services for health and related areas for families of CSHCN. This program is a referral linkage system to provide for those children ages birth to 21 who have, or are suspected of having, a serious or chronic physical, developmental, behavioral, or emotional condition and who require a health or related service of a type or amount beyond what is required by children generally. Physically Handicapped Children’s Program
The CSHCN Program also offers financial assistance under the Physically Handicapped Children’s Program (PHCP). The PHCP assists families to pay medical bills for children with severe chronic illnesses and/or physical disabilities.
The PHC Program pays for Diagnosis and Evaluation, and Treatment Program.
