New York State Care at Home (CAH I/II)

Program Goal

To enable physically disabled children to remain in the home setting.


  • Must be under 18 years of age.
  • Require the level of care provided by a skilled nursing facility or hospital.
  • Must be able to be cared for at home safely.
  • Must be eligible for Medicaid based on his/her parent’s income and, if applicable, resources;
  • Must be ineligible for Medicaid due to the parents’ excess income and/or resources, but be eligible for Medicaid when parents’ income and/or resources are not counted.

In addition: In order to be eligible for Care at Home (CAH), the child must not be married.


The Medicaid-waiver CAH I/II program provides community-based services to physically disabled children who require hospital or skilled nursing home level of care, and allows the child to be at home instead of in an institutional setting. Medicaid state plan services and waiver services, including case management, respite, home adaptations, vehicle modification and palliative care are available to assist families in caring for their disabled child at home. Considered as a group, the cost of care of children participating in CAH I/II can not exceed that if cared for in a skilled nursing facility or hospital.
