Community Based Services (CBS)

Community Based Services provides people with autism and other disabilities the best quality of life through residential and community-based opportunities. CBS (Community Based Services) meets the changing needs of children, teens, and adults through a customized blend of individualized services. Our programs are creative, innovative, and designed with your loved one’s interests and goals in mind. The CBS direct care team is committed to a standard of excellence that encompasses innovation, collaboration, and knowledge. With a focus on person-centered care, CBS supports families in the Mid-Hudson Valley region of New York. The agency has been enriching lives, providing jobs, and cultivating dreams for people with disabilities since 1981.

While we provide the highest level of individualized care for children and adults with disabilities, we are a different kind of agency, because we infuse our Day Hab, Community and Respite services with an added layer of innovation, that brings joy and creativity to everyone we serve. Take our newest innovation — the Hudson Valley InterArts Center, which is fast becoming a vibrant arts hub in Patterson, NY, and beyond at our satellite site at Cultivating Dreams Farm. People enrolled in CBS services can engage for a half or a full-day immersive experience in music, dance, arts and crafts, pottery, and yoga. Classes are offered six days a week. Be part of a community of artists who share the joy of creating in a safe and supportive environment. Through pairing mindfulness and meditative arts with creative arts, the InterArts Center also aims to support social and emotional health. We believe artistic expression and creativity can elevate each unique voice and story to enrich community for all.

Cultivating Dreams Farmhttps://cultivatingdreams.org/
People come to Cultivating Dreams Farm to invest in themselves and their dreams. Set on a small farm in Dutchess County, people of all abilities engage in agricultural and life skills. At Cultivating Dreams, they blossom into confident young adults with a sense of purpose.

People supported by Community Based Services can experience a year-round agricultural experience in our two greenhouses and open-air gardens. They gain pre-vocational skills by learning the basics of planting, raising, and harvesting simple crops. Life skills, such as cooking and home maintenance, are taught in the farmhouse. The expansive field, basketball, bocce and pickle ball courts, and weight room support the wellness program. Creative art classes are held in the renovated barn. In addition to agricultural, life skills, wellness and art activities, participants impact the local community through volunteer opportunities with Dutchess Outreach, the Food Bank of the Hudson Valley, and local senior centers.

As part of its mission, Community Based Services creates innovative ways to provide jobs for people of all abilities. For the people we support, employment means greater self-sufficiency, the opportunity to apply skills and greater inclusion in the community. Two of our social enterprises are Good Reasons Dog Treat Bakery and GR Pack, a fulfillment and packing company.

 

CBS Services
Housing/Residential Services
CBS provides housing for adults with disabilities throughout Westchester County in 10 Individual Residential Alternatives or IRAs. These assisted living sites provide 24-hour support within a safe and comfortable home and family environment. Each home is equipped with bedrooms and accessible bathrooms, kitchens, communal areas, and backyards for gardening, grilling or relaxation. Our highly trained staff strive to promote personal choice, safety, and enjoyment of life for the residents. Each person living in the residence helps plan his or her community-based activities, such as shopping, entertainment, and social interests. In-home Day Habilitation support is also available to those interested in activities promoting community engagement.

Home and Community Services
Community engagement is an important part of living a full and enriched life. The At Home Services – Community Habilitation program provides one-on-one support so that people can be more independent in the home and in the community. Goal-oriented activities promote skill building and support the Life Plan. CBS can assist families in finding staff or CBS can also hire a family’s trusted person, so that he or she can be compensated for their time.

In-Home Respite
In-Home respite care for children and adults with autism and developmental disabilities provides experienced and compassionate staff to give you a break from caregiving. While you are using the time for self-care, trained staff will attend to the physical, safety, social and recreational needs of your loved one within your home.

Site-Based Respite
CBS also offers a choice of three different Site-Based Respite locations at our Cultivating Dreams Farm in Hopewell Junction, in Peekskill or at our Hudson Valley InterArts Center in Patterson, NY. Respite offers a much-needed break for families by providing a holistic and enjoyable experience for their loved ones.

Site-Based and Community Focused Day Services
People can spend the day with friends and staff in our certified day habilitation site in North Salem, NY.  There they can engage in projects, work on computer skills in the media center, or wind down in the relaxation station equipped with massage chairs. From our hub, they may also be offered opportunities to explore the community during activity trips.

Community Focused Day Habilitation without Walls helps people reach their goals by being in the center of the action, right in the community. They have an impact on the community by growing vegetables on a farm, training service dogs, delivering meals to the elderly, and collecting food for the hungry.  Participants will develop friendships and work skills along the way. In addition, our 10-acre Cultivating Dreams activities center is also available for life skills, physical fitness, and agricultural activities. Or engage in a half-day or full-day immersive experience in the arts, dance, and music at the Hudson Valley InterArts Center.

Employment Services
For those excited to enter the world of work, CBS offers pre-vocational training, paid internships and supported employment with job coaching. Each person receives a personalized employment plan and the skills needed for successful job placement in a competitive environment.

Transition Planning Services
Today, students have big dreams and even bigger goals for their adult lives after high school.  For students with disabilities, transition planning is an important part of reaching these goals.  The CBS transition planning team works with families to design a plan and be equipped to prepare for, navigate and find suitable support and services for their child.
